Introduction:
This Service Agreement (“Agreement”) is entered into between NOVOMED MEDICAL GROUP, INC., A PROFESSIONAL CORP (“Provider”) and the undersigned patient (“Patient”), for Get Virtual Care Now service. By signing this Agreement, the Patient acknowledges and agrees to the terms, services, payment obligations, and policies outlined below.
Payment Options
- Please complete payment prior to appointment on novomed.care
Refund Policy
- All service fees are non-refundable if canceled by the member. If a visit or service is canceled by the provider, any refund will be issued at the sole discretion of the medical group.
- Patient agrees to pay regardless of the treatment of the outcome
Patient Responsibilities
The Patient agrees to:
- Provide updated contact and payment information
- Use provided communication systems for non‑urgent medical needs
- Provide accurate health history and information
- Follow care recommendations as appropriate
Provider Responsibilities
NOVOMED MEDICAL GROUP, INC., A PROFESSIONAL CORP agrees to:
- Provide service to patient
- Maintain secure communication and protect patient health information
- Notify the Patient of any material changes to services or policies
Insurance and Deductibles
Patients are responsible for all charges applied to their deductible. NovoMed Concierge Medicine does not cover or waive deductible amounts. Any portion of services not paid by your insurance plan (including deductible responsibility) will be billed directly to you.
Insurance Billing and Third-Party
Iris Health LLC is the MSO that provides administrative and management services to NovoMed Medical Group and is not responsible for the provision of medical care. NovoMed Medical Group does not submit claims to PPO or commercial insurance plans. Any covered services that are eligible for insurance reimbursement will be billed through Blossom Ridge Medical, Inc., a separate entity that is independently contracted with applicable insurance carriers.
